Once you begin the nomination form you will not be able to save and return to the form later. Please be prepared to complete the entire nomination at one time. The online nomination form requires:
o Category of the nomination
o Nominator (person completing the form) and nominee contact information
o Full name
o Phone number
o Contact information for three references who are familiar with the nominee's service.
o References cannot be the nominator
o References cannot be a member of the nominee’s immediate family (i.e. spouse, significant other, mother, father, sister, brother, or child)
o Narrative Sections on Achievement, Need, Innovation, Mobilization, and Endurance
o Achievement: Please describe how the nominee's accomplishments are exemplary, far exceed expectations, and are measurable. (2,500 character limit)
o Need: Please describe how the nominee addresses a need that was not previously met in the community. (1000 character limit)
o Innovation: Please describe how the nominee applies a new solution to an existing social problem. (1000 character limit)
o Mobilization: Please describe how the nominee empowers the community to support the program/event as demonstrated by the quantity and/or quality of volunteers. The service should result in opportunities for mobilizing other volunteers. (1000 character limit)
o Endurance: Please describe how the nominee develops a long-term solution to social issues through volunteer service. The service must be direct and ongoing, rather than a one-time activity, with the exception of crisis situations. (1000 character limit)
o The Volunteer Manager nomination form has an additional narrative section. In addition to the above information, will need to describe how the Volunteer Manager nominee exemplifies these best practices in volunteer management (1000 character limit):
o Mission-based alignment of volunteer engagement
o Program evaluation and continuous improvement
o Strategies to recruit a diverse volunteer base
o Development of clear volunteer position descriptions
o Consistent use of a screening and interviewing process
o Orientation and training for volunteers
o Appropriate supervision of volunteers
o Recognition and retention of volunteers
o Appropriate policies and procedures
We suggest nominators use a word document to prepare and record answers to fields, so if something does happen you will have a saved copy of the responses to each field.
